Samuel's Charity At Home Project is looking for a Band 6 nurse be part of the At Home project in South London. The At Home Project is part of the Community Child Nursing team in Wandsworth with the core focus of the role enabling children to leave the wards and to support them, eliminating unnecessary re-admission and increasing the level and quality of their care.

Job Title: Band 6 Care Co-Ordinator Project Manager, At Home Project, S London

Responsible to: Chief Executive, supported by Development Director.

Responsible For: Project Launch and Development of the Samuels Charity “At Home” project, a caseload of 20 children/200 beneficiaries p.a.

Hours of work: 25 – 37.5 hours per week, worked flexibly to suit the service profile and needs of the Charity. At least 4 days per week.

Wage: Circa £48,000 based on experience. Pro Rata if below 37.5 hours

Purpose of role:

Samuels Charity is a small Charity with big ambitions. This new role represents a significant development for the Charity and the post will mark a substantial step forward in its service profile and resources.  It's a great opportunity to make a big difference and play a significant part in the development of much-needed services and support. .

  • To support the CEO by launching the At Home Project in Wandsworth/S London and developing it in ways that are relevant and in line with our Charitable Objectives and Business Planning.
  • To work with the rest of a small but high quality team to help develop the service offering of the charity and to specifically develop the At Home Project as a template for replication and expansion.
  • To develop partnership working , especially with the St Georges Hospital Team and to support better integration of transition care for seriously and terminally ill children.
  • To be a key team member in a small but ambitious and growing organisation doing amazing and much needed work. To help ensure the Charity fulfils its operational and fundraising plan effectively.

Key Responsibilities:

  • To provide assessment, emotional support and guidance to children and families on a case by case basis and to manage a caseload of around 20 children at any one time, approximately 200 children per year.
  • To support the launch and development of the At Home Project in Wandsworth, South London.
  • To work as a key member of the Community Child Nursing team in ensuring the smooth running of the organisation day to day and its integration of this project in its service offering.
  • To work with the Hospital Team and to act as a catalyst and support to better transition support for children moving between Hospital and Home settings, between or after bouts of care.

Co-ordination and management

  • To ensure accurate and accessible systems, and records, including support and progression markers and relevant communications with all stakeholders.
  • To develop a local volunteer team of trained mentors to expand the scope and level of care we can provide. To support the design of such provision for effectiveness.
  • In time, to work with the CEO to develop peer support mechanisms for parents, to help address the support gaps such parents/carers face when their child is so ill.
  • To support partnerships working and development across all stakeholders and potential partners, including local authorities, NHS staff and the business and wider community.
  • To support effective marketing for the charity, including providing inputs for social media and other marketing (led by the CEO).
  • To research and explore options for development, working especially with the CEO and Development Director.

Administration/marketing

  • Provide timely reports as required both to the CEO & Board of Trustees.
  • Ensure excellent administration, record keeping and reporting as required, including data and database
  • Maintain communication logs, transferring and utilising data as appropriate and ensuring any relevant deadlines are achieved.

 

Partnership and Liaison

  • Work closely with the team, and with key stakeholders to promote good communication and shared focus.
  • Ensure effective, relevant and appropriate communication at all times
  • Represent the Charity professionally and appropriately to other organisations, funders and supporters and develop those relationships
  • Help establish and develop good relationships across our key stakeholders and with other charities and agencies to increase partnership and effective joint working where appropriate.
  • Be an effective point of contact for team members and external contacts as regards activities and service provision of the project.

Samuels Charity is a registered charity Limited by Guarantee, and relies heavily on fundraising for its income. Staff and Trustees are expected to be an extension of the fundraising team and to support occasional and especially key fundraising events and to engage their own people networks in gaining support for the Charity.

  • Promote and foster the reputation and standing of Samuels Charity at all times and support the rest of the team in doing the same.
  • Undertake and identify training as required and take a positive approach to personal development.
  • Undertake any other reasonable tasks deemed necessary
